PATH (People Assisting the Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness since 1984. We operate across California providing street outreach, interim housing, case management, and permanent supportive housing.
Position Overview
Join PATH's Health - PSS Team in Los Angeles as a CalAIM Enhanced Care Management Case Manager. You will focus on member medical care coordination, assist in navigation of the healthcare system, and ensure medical and housing stability for high-acuity CalAIM participants.
Responsibilities
- Conduct intake assessments and develop individualized service plans addressing housing, income, medical, mental health, and social support needs
- Provide referrals and linkages to healthcare, housing, and community support resources
- Complete program-specific assessments for CalAIM Enhanced Care Management
- Coordinate with medical providers to support healthcare navigation
- Maintain accurate, HIPAA-compliant documentation and case file accuracy
- Conduct 90-day progress reviews with participants
Minimum Qualifications
- Employment authorization to work in the U.S.
- Valid California driver's license, reliable vehicle, and proof of insurance
- CPR/First Aid certification (or willingness to obtain upon hire)
- Must pass tuberculosis test, background screening, and drug test
Preferred Qualifications
- 2+ years of experience in healthcare, case management, homeless services, or vulnerable population support
Compensation
$23.24 – $28.28 per hour | Full-Time, Non-Exempt | 24 pay periods/year
