PATH (People Assisting the Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness since 1984. We operate across California providing street outreach, interim housing, case management, and permanent supportive housing.
Position Overview
Join PATH's Care Coordination Services team in Orange County. As a Case Manager, you will assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach, supporting people across all housing phases from street outreach through permanent housing placement.
Responsibilities
- Conduct intake assessments and develop individualized service plans addressing housing, income, medical, mental health, and social support needs
- Provide referrals and resource linkages to help clients achieve their goals
- Complete program-specific assessments (VI-SPDAT)
- Conduct 90-day reviews and maintain comprehensive case documentation
- Collaborate with housing partners, health providers, and social service agencies
- Maintain HIPAA-compliant case files
Minimum Qualifications
- Employment authorization to work in the U.S.
- Valid California driver's license, reliable vehicle, and proof of insurance
- CPR/First Aid certification (or willingness to obtain upon hire)
- Must pass tuberculosis test, background screening, and drug test
Preferred Qualifications
- 2+ years of experience in healthcare, case management, homeless services, or vulnerable population support
Compensation
$23.24 – $28.28 per hour | Full-Time, Non-Exempt | 24 pay periods/year
