ICMS Case Manager

About the Role
Intensive Case Management Services (ICMS) Case Manager working with formerly homeless individuals who have chronic medical conditions and co-occurring disorders. Focuses on housing retention through strength-based, trauma-informed case management using motivational interviewing and harm reduction approaches.
Salary: $23.69 – $27.87/hour | Full-time (40 hrs/week)
Location: Long Beach, CA

Key Responsibilities
- Collaborate with referring agencies (VA, Housing Authorities) and property management
- Connect clients to mental health, substance abuse, employment, and housing services
- Provide strength-based case management and service coordination
- Conduct assessments, individual and group sessions, and crisis interventions
- Maintain HIPAA-compliant documentation and progress notes
- Attend client appointments (up to 50% travel required)
- Provide evening and weekend coverage as needed
Requirements
- Minimum 2 years' experience working with homeless populations
- Case management or mental health services background
- Proficiency with HMIS, MS Word, and Excel
- Valid driver's license and reliable transportation
- Background check and fingerprinting required
