About the Role
Intensive Case Management Services (ICMS) Case Manager working with formerly homeless individuals — including transitional youth — who have chronic medical conditions and co-occurring disorders. Uses harm reduction, critical time intervention, and stages-of-change methodologies to help clients maintain housing and manage complex needs.
Salary: $23.69 – $27.87/hour | Full-time (40 hrs/week)
Location: 11815 Wilmington Avenue, Willowbrook (Los Angeles), CA

Key Responsibilities
- Coordinate with housing authorities, property management, and community partners
- Connect clients to mental health, substance abuse, employment, and housing services
- Provide strength-based, trauma-informed case management
- Conduct assessments, individual and group sessions, and crisis interventions
- Maintain HIPAA-compliant documentation and progress notes
- Travel to client sites (up to 50% of time)
- Work flexible schedules including evenings and weekends as needed
Requirements
- Minimum 2 years' experience working with homeless populations
- Crisis intervention experience
- Strong written and verbal communication skills
- Valid driver's license and reliable transportation
- Background check and fingerprinting required
