The role /span span class= NormalTextRun SCXW151518344 BCX0 facilitates /span span class= NormalTextRun SCXW151518344 BCX0 linkage to community resources and 3rd party contracted partners as needed for residents. /span /span span class= EOP SCXW151518344 BCX0 data-ccp-props= { 134233117 :true, 134233118 :true, 201341983 :0, 335557856 :16777215, 335559740 :240} /span /p p strong Responsibilities: /strong /p ul li Deliver onsite programming at assigned properties, ensuring alignment with internal standards and regulatory requirements. /li li Conduct door-to-door, phone, email, and other outreach strategies to promote awareness of available services. /li li Provide crisis support and follow mandated reporting procedures for incidents such as child or adult abuse. /li li Support residents with functional impairments through direct case management and development of independent living skills. /li li Facilitate group and individual services, fostering community engagement and supporting residents’ personal and professional goals. /li li Collaborate with third-party partners and local agencies to deliver, manage, and evaluate effective services. /li li Maintain accurate records, ensure timely reporting, and uphold HIPAA and mandated reporting standards. /li li Assess and respond to resident needs with empathy and professionalism, contributing to the development of responsive service plans. /li li Participate in team meetings and events, model a positive attitude, and maintain strong relationships with property management and colleagues. /li /ul p strong Qualifications: /strong /p ul li Strong reading, writing, and verbal communication skills; comfortable navigating language barriers using translation tools. /li li Demonstrated judgment, discretion, and problem-solving abilities when working with diverse resident populations. /li li Skilled in relationship-building, active listening, and conflict resolution. /li li Highly organized with attention to detail; able to work independently and collaboratively within a team. /li li Familiarity with mental health diagnoses and interventions, including harm reduction, motivational interviewing, and trauma-informed care. /li li Committed to ongoing learning and staying current with best practices in social services for special needs populations. /li li Proficient in Microsoft Office Suite, Outlook, OneDrive, Teams, Zoom, Salesforce, and other relevant technology platforms. /li li Must pass background and fingerprint checks; possess a valid California Driver’s License, current auto insurance, and reliable transportation. /li li Ability to travel between properties /li /ul p strong Education and Experience /strong /p ul li span data-contrast= none M.S.W. or M.A. degree in psychology, counseling, or related field and 1 year of full-time experience serving high needs populations (mental illness, substance use, co-occurring disorders) or B.A. or B.S. degree in the same fields with 3 years full-time experience serving these populations or a High School Diploma or GED plus 5 years or more of relevant experience /span span data-ccp-props= { 134233117 :true, 13423311
