Location: San Mateo $28.85 per hour Organization Overview Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. We implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services, and health care sectors. Department Summary Brilliant Corners partners with Health Plan of San Mateo, San Mateo County, and other community partners to provide person-centered housing services to members of the health plan. Brilliant Corners helps members identify housing opportunities in the community that meet their needs and assist them with the application process. They also assist members with applying for waitlists for rent subsidies and affordable housing. They help members move into housing and provide ongoing support with their tenancy. Brilliant Corners administers rent subsidies to qualified members. Position Summary With guidance from the Program Supervisor, the Senior Housing Coordinator will provide housing support and coordination, move-in coordination, reasonable accommodation request reviews/support, Salesforce auditing, Housing Coordinator training manual development/updates, and housing retention services to participants. In addition, the Senior Housing Coordinator may take point with coordinating monthly meetings with property providers/management, facilities and/or case management. The Senior Housing Coordinator will maintain a caseload of 30 clients and will work directly with tenants and ongoing case management to ensure that tenants successfully retain their housing over time. They will ensure tenant well-being and unit habitability by conducting weekly, monthly, and quarterly visits and will work creatively and collaboratively to prevent eviction and other housing-related issues. Position Responsibilities Collaboratively work with Service Providers, Property Management and/or Facility Administrator/Staff on all resident issues with support from the Program Supervisor. Coordinate regular meetings with partner agencies and take the lead on facilitating the meetings on behalf of Brilliant Corners. Coordinate move-ins Act as a liaison between clients, Service Providers/Case Management partners May assist with various trackers like exits and move-ins May assist with housing placements and assignments Review and provide support for reasonable accommodations Audit Salesforce data Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks. Assist with developing/updating Housing Coordinator training manual and onboarding new staff on the team Respond to housing and tenant-related emergencies during normal business and after hours. Maintain a caseload of about 30 clients Become competent with the housing rights of people with disabilities under Section 504 of the Rehabilitation Act of 1973 and the Fair Housing Act, As Amended in 1988 Assess the need for reasonable accommodations/unit modifications and ADA compliance, and assist program participants through the process of obtaining the request Educate community members about the housing needs of identified program participants and the importance of supportive housing Submit and follow up on tenant maintenance requests Maintain thorough and accurate progress notes, files, and correspondences while maintaining the confidentiality of tenants, staff and organizational information at all times and exercise appropriate boundaries with tenants Routinely make home visits and phone calls to program participants Attend all agency staff meetings and trainings, as well as department meetings and case conferences Other duties as assigned Requirements Professional Experience Experience presenting in front of a large group of people Experience with interpreting data and running reports Knowledge, Skills, and Abilities Willingness to travel and make multiple visits in the field Experience working with homeless individuals Highly organized and reliable Able to take feedback well Experience working in a crisis setting and ability to respond appropriately in an emergency Knowledge of supporting housing Knowledge of Fair Housing Laws and reasonable accommodations Excellent verbal and written communication skills Strong interpersonal skills Possession of a valid California driver’s license, a clean driving record, and automobile insurance Access to reliable transportation Basic computer knowledge, MS Word and Excel required; PowerPoint preferred Flexibility required regarding scheduling and prioritizing of tasks Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment Ability to work independently and collaboratively Core Competencies Approachability: Builds trusting relationships by being accessible, understanding, and responsive to others needs to improve services Conflict Management: Addresses conflicts by understanding needs, employing empathetic listening, identifying underlying issues and facilitating equitable solutions that respect all parties Strategic Agility: Future-oriented, strategically coordinates resources and stakeholders to optimize support, proactively navigating challenges through planning and critical thinking Organizational Values Humanity: Putting people first: We are committed to meeting people where they’re at, honoring their dignity, diversity
